Acknowledgment statement
I acknowledge that I have received a copy of the [Company name] Employee Handbook (version [X], effective [date]). I understand that it is my responsibility to read the handbook and to ask my manager or HR if I have questions about any part of it.
By signing, I confirm that:
- I have received access to the current employee handbook
- I understand it is my responsibility to read and follow its policies
- I understand the handbook is a guide and does not create an employment contract
- I understand the company may update the handbook, and the current version always applies

Frequently asked questions
Why collect a separate acknowledgment form instead of just the handbook?
A short, standalone acknowledgment is easier to file, sign electronically, and re-collect on every update than re-signing the whole handbook. It also creates a clean, dated record that the employee had access to that specific version.
Do we need to collect a new acknowledgment every time we tweak the handbook?
For minor wording changes, usually not. For material policy changes — a new leave policy, a changed conduct standard, updated compliance language — re-collect it so you have a current record that employees were notified.
Does signing this form make the handbook a binding contract?
No, and the acknowledgment statement says so directly. The form only confirms the employee received and understands the handbook, not that the handbook itself is a contract of employment.
